    Finding a new dental office can often be challenging. It's our mission to make that search easier for you. We provide servies for childen ages 0-18.

  • New Patient Paperwork

    We will send you a message with a link which you can use to fill out our new patient forms before your appointment. If not completed before your appointment we ask that you arrive early by at least 30 minutes the day of your appointment to fill out the forms here.

  • Records Release Form

    This form is needed for us to share patient records with your other health care providers.

    We have made it easy for you to download the Records Release Form. Please print out, sign, and bring this with you to your next appointment.

    Appointment Reminders

    It can be easy to forget when you scheduled a dental appointment but now we're able to remind you with an Appointment Reminder via phone call, text message and/or email.

Dental Treatment Cost

Every person is unique; therefore, their dental issues or treatment plans will also vary. The cost of treatment depends on the treatment plan, which is set according to the concerns and needs of one's oral health.

Before starting treatment, the Purple Tooth Dental Team will discuss with you the treatment cost and available payment plan options, so that you can choose the best option and take any stress off the treatment expense.

Insurance

We will assist you to prepare your claim to submit to your contracted dental insurance.

If you do not have insurance or aditional assistance is needed we offer additional payment options including an in house Dental Membership Plan as well as services such as CareCredit.
